Office Policy and Hours

Office Hours

Scheduled Appointments

Patients should arrive 15 minutes prior to their actual scheduled appointment time.  This is to verify each patients address, phone numbers, and insurance information along with any forms that need to be completed.  Please bring all medications to each visit or a complete list with the medication name and dose taken.  Request that parents only schedule two siblings on the same day.  We understand that delays can happen, however, we must try to keep the other patients and providers on time.  If a patient is 15 minutes past their scheduled appointment time, we will have to reschedule the appointment.

Cancellation/No Show Policy

Our providers make every effort to provide prompt medical care to all patients and their appointment slots are limited.  If you are unable to keep a scheduled appointment, please let us know 24 hours in advance.  This ensures an opening for other patients that may need medical care.

If contact is not made and the appointment is missed this is called a NO SHOW.  A NO SHOW will generate a $50.00 fee that insurance does not cover.  If two no shows occur within a year, the patient will be placed on an alternate schedule.  The alternate schedule has limited availability.  If five no shows occur within a year, you may be terminated from receiving services at out practice.  If a chronic condition visit (ADHD or Asthma, initial or recheck) is missed, you will go on a monthly schedule for 3 months.  If good attendance is kept for 6 months, you can be placed back into the provider’s schedule.

We understand that there may be issues beyond your control and want to be understanding of special circumstance.  Please contact our office manager in those situations.
